I have been requested to take pictures of a 15 yr old model. The pictures are going to be similar to the following. What kind of model release document I should use for this (I guess, legally, the model cannot sign the document)? Can model's parents sign for such documents? If so, should I ask one parent or both of them to sign?
Many model releases have a space at the bottom for the signature of the parent or guardian. Be sure to check ID of all parties. Also be sure that you have someone with you, and the parent or guardian should also have some one at the shoot.
Use a MRF suited for a minor, most MRF's have a bottom section for minors. The minor cannot sign, it would have to be signed by a parent or legal guardian. Have either one (parent/legal guardian) present on the day of the shoot.
Then use your common sense in ref to where you post such pics.
Its not clear from your question "I've been requested to shoot...." whether you are shooting the photos for commercial use, or a family portrait, or for her to get into modeling. If its for the commercial use, you would need a release (have the parent sign, as the others have said). If the other two, you don't need a release, unless you plan on using the photos yourself, even for self-promotion. If only for self-promotion, you can write that into the release and have the parent sign that clause too. No need to make them stress over a complete "killer" release if you only need the pix for your own self promo.

Andy,
Well... here is the story (very short). I normally do not take pictures of models less than 18 years for TFP. However, one of the models who did TFP with me has a 51 yr old sister who wants to get into modelling. So, she wants some pictures for self promotion. I might post some of those in this forum to critique. Other than that, they are of no use for me. I don't really care whatever she does with the pictures. I just wanted to avoid any "concerns" after the shoot (to be on the safe side).
